Financial Assistance Program
The Grace Health System® Financial Assistance Program helps to make our health care services available to everyone in our community needing emergent or medically necessary care. This includes people who do not have health insurance and are unable to pay their hospital bill, as well as patients who do have insurance but are unable to pay the portion of their bill that insurance does not cover. In some cases, eligible patients will not be required to pay for services; in others, they may be asked to make partial payment.

Request for Medical Records
Requests for medical records must be made in writing. Download and complete the Authorization to Use and Disclose Protected Health Information Form.
We help keep the lines of communication between you and your doctor open with our Patient Portal. Here you can find everything from lab results to notes from your doctor. You must have access code from your doctor.
Each new or established patient is asked to fill out a Patient Care Form prior to seeing a physician. This document will allow your doctor to know your complete medical history to best serve your health care needs. It only takes a few minutes but makes difference in the quality of your care.
